For several years, The Leapfrog Group has asked hospital to report on an NQF-endorsed measure, Medication Reconciliation: Number of Unintentional Medication Discrepancies per Medication (NQF 2456) that calculates the rate of unintentional medication reconciliation discrepancies among admission orders and discharge medication orders. 

This learning lab will focus on how hospitals can engage their pharmacists and certified pharmacy technicians in implementing the measure, using best practices to collect a “gold standard medication history,” identifying unintentional discrepancies, and using the data collection to close gaps in the medication reconciliation process (i.e., errors in history and errors in reconciliation).

Learning objectives:

• How to obtain a ‘gold standard medication history’

• How to identify unintentional discrepancies among admission and discharge orders

• How to use the data collected to communicate issues back to clinicians obtaining the pre-admission medical list and reduce/prevent unintentional medication discrepancies
